Hospice care provides medical, emotional, and spiritual support for individuals with life-limiting illnesses and their families. According to Medicare guidelines, hospice care is appropriate when a physician certifies that an individual has a life expectancy of six months or less if the illness follows its natural course.

Where Hospice Care Happens?

One of the biggest questions families ask is: “Where will my loved one receive care?” The answer is simple: wherever they’re most comfortable.

Hospice care is provided in:

Who Qualifies for Hospice Care?

Hospice is available to anyone with a life-limiting illness when curative treatment is no longer effective or desired. Common diagnoses include:

  • Cancer (all types)
  • Congestive heart failure (CHF)
  • Chronic obstructive pulmonary disease (COPD)
  • Dementia and Alzheimer’s disease
  • End-stage kidney or liver disease
  • ALS (amyotrophic lateral sclerosis)
  • Stroke with severe complications
  • Parkinson’s disease

What Our Hospice Care Includes?

James River Home Health & Hospice brings together medical expertise, personal care, and emotional support; all coordinated by one team and covered by Medicare, Medicaid, and most insurance plans. Here’s what that looks like:

  • Medical Management- Physician-led pain and symptom control, with our Medical Director overseeing your care plan and registered nurses providing 24/7 clinical support.
  • Personal Care- Certified nursing assistants and CNAs/aides provide bathing, dressing, and personal hygiene assistance with dignity and respect.
  • Emotional & Spiritual Support- Licensed social workers and chaplains help with emotional processing, advance planning, and spiritual care tailored to your family’s beliefs.
  • Family Support– We teach you what to expect, how to provide care, and how to navigate difficult decisions with confidence.
  • Social Care– Licensed social workers assist with care coordination, community resources, advance directives, and family counseling during this difficult time.
  • Volunteer Support– Trained volunteers offer companionship, respite for caregivers, and light assistance to give families extra support.
  • Bereavement Services– Grief counseling and support continue for up to 13 months after your loved one passes.

The Four Levels of Hospice Care

Not every day looks the same. Medicare recognizes four levels of hospice care, and we adjust as your loved one’s needs change:

  • Routine Home Care Regular visits from nurses and aides, with 24/7 on-call support. This is the most common level of care.
  • Continuous Care Short-term, intensive nursing care at home (up to 24 hours a day) during a symptom crisis to avoid hospitalization.
  • Inpatient Respite Care A brief stay (usually up to five days) in a contracted facility to give family caregivers a rest.
  • General Inpatient Care Short-term inpatient care in a hospital or hospice facility for acute symptom management that can’t be handled at home. Our team manages transitions between levels seamlessly so you never have to navigate this alone.

I had to undergo surgery in April and James River Home Health was assigned to me by Spotsy Regional Medical Center. I feel blessed to have been cared for by Nikki and Kate. They were both friendly, knowledgeable, professional and showed great concern for my overall health, recovery and treatments. This morning [6/1/2026] was my last visit from Nikki and I was released from care. They were never more than a phone call away and when I needed help, they were able to calm my fears of the infusion equipment and instruct us in how to overcome the issue at hand. I recommend them highly as a partner in your recovery journey.
Since I’ve had the opportunity to be on the receiving end of the James river hospice family, I want to say most everyone has been amazingly supportive and helpful. However, one stands out ginormously Crystal Singleton has went out of her way on so many occasions and on so many levels to make sure my Aunt Rosa was taken care of. She never rushed her always wanted to know if I needed her to do more. It didn’t matter the mood my Auntie was in Chrystal’s christ like character was always love. I was given 2 weeks (twice a week) of PT at home after my knee surgery-such a blessing! The day after surgery, James River Home Health came to my home and started PT. My therapists were Mark and Brandon. They were the best. Not only for my PT but they knew exactly all the struggles I was facing in the weeks to come and were so knowledgeable. They completely put my mind at ease despite the pain I was going through. At 3 weeks, I went to Ortho VA for the rest of my PT. The staff were VERY impressed with the progress I had made. No wheelchair, cane or walker needed! At 4 weeks, I was able to reduce my visits to once a week. Now, I am 6 weeks out and am due to be discharged! I know I owe this to Mark and Brandon for the great start they gave me at home to put me way ahead of the healing process. Thank you guys!!! HIGHLY RECOMMENDED

Our experience with physical therapy through James River Home Health was very positive. The care provided was thorough, supportive, and well-suited to my mom’s needs, especially considering she has Alzheimer’s. Ian, the physical therapist, was excellent. He showed a great deal of patience, kindness, and understanding during each visit. He took time to explain things clearly, worked at a pace that was comfortable for her, and created a calm, reassuring environment. His approach made a meaningful difference in how she engaged with therapy. From a PT perspective, I have no complaints. With Alzheimer’s my mom cries a lot but whenever she had PT with Ian her mood was different when he left.

My grandmother’s experience with Ian, the physical therapist has been amazing. He is the epitome of a clinician who not only treats the outer but the inner of a patient. Often she calls me to rent and rave about her conversations with him and how good he made her feel and how he encouraged her and how she really enjoyed therapy with him. Ian Ian listens to my grandmother. He encourages her, but also he pushes her because he knows that she’s capable of more five stars across the board.

The best part of this experience with James River is when Ian came to help my mother with regaining her mobility. He was very supportive, encouraging and motivating. He honestly went over and beyond to get her the DME ( Durable Medical Equipment) she needed. Ian honestly became apart of our family and it was hard for my mom when the services ended. With older people it is hard for them to trust people in their homes but Ian worked his magic quickly and also became a Companion. Thank you very much Ian for taking great care of my mother.
