Skilled Nursing at Home
Skilled nursing is the clinical foundation of home health care. Our team of Registered Nurses (RNs) and carefully selected Licensed Practical Nurses (LPNs) visit patients at home to manage the medical needs that make post-hospital recovery so challenging.
Skilled nursing visits may include:
- Medication management – reviewing medications, monitoring for side effects, and educating patients and caregivers on proper use
- Wound care – professional assessment and treatment of post-surgical or chronic wounds
- Vital signs monitoring – tracking blood pressure, heart rate, oxygen levels, and weight to catch changes early
- IV and infusion site care – managing infusion therapy and central line access in the home
- Feeding tube care – supporting patients who require enteral nutrition
- Patient and caregiver education – plain-language guidance on managing conditions like diabetes, heart failure, and COPD
Our nurses do not just check a box. They take time to understand what is happening at home and make sure you feel confident managing between visits.
What to Expect When You Start Services
- Reach out or receive a referral. Your doctor, hospital discharge planner, or care coordinator can refer your loved one directly to us. You are also welcome to call us at 1 (855) 415-5744 to ask questions before a referral is placed.
- We complete a home visit and assessment. A registered nurse will visit the home, review your loved one’s medical history and current needs, and coordinate with the ordering physician to build a personalized plan of care.
- Care begins on your schedule. Your care team will establish a visit schedule tailored to your loved one’s needs. We are available 24 hours a day, 7 days a week – including nights, weekends, and holidays – should a concern arise between.
