Specific emphasis was placed on our specialty programs to reduce re-hospitalizations. We have a proven track record and area hospitals work closely with us for this reason. The single most important piece of these programs is patient participation.
Orthopedics
Protocols designed by the surgeon or treating physician are on file or sent by the hospital to follow exactly what your physician wants our clinicians to do in your recovery from an injury or surgery. We are specialists in joint replacement and spinal surgeries.
Respiratory Care
This program is specifically designed to treat moderate to chronic respiratory illnesses such as chronic obstructive pulmonary disease (COPD). We have a specific set of protocols designed and proven to reduce readmissions and improve quality of life. We work very closely with the largest area pulmonary practice and have unique access to their services.
Cardiac Care
The Cardiac Management Program is designed to help patients recover from cardiac events and reduce the number of recurring events through the management of symptoms, diet, exercise, and medication. We work with your cardiologist post hospitalization to design a treatment regimen to prevent re-hospitalization. The leading disease causing re-hospitalization is congestive heart failure (CHF), and our program is proven to reduce readmissions below the national and local averages.
Neurological Diseases
Neurological disease management programs at James River Home Health stand on a solid foundation of neurological sub-specialized clinicians. We have expert clinicians in the following: stroke/cerebrovascular accident (CVA) care, Parkinson’s disease management, spinal cord injuries, multiple sclerosis care, amyotrophic lateral sclerosis (ALS)/Lou Gehrig’s disease management, and dementia and Alzheimer’s disease care.
Palliative Care
Our palliative care program is an approach that improves the quality of life of patients and their families facing the problems associated with life-threatening illness, through the prevention and relief of suffering and other problems—physical, psychosocial, and spiritual. Think of palliative care as a bridge between hospice and home health care. We offer full-service hospice care through our sister organization, James River Hospice, when patients and families are ready to transition. Patients may choose to return to home health care and are not required to stay in hospice if they decide against it. Please review our Hospice vs Home Health Care document to aid in this discussion or talk to one of our clinicians to see if hospice is the right thing for you or your loved one.
